Notes:

• All requests will be manually reviewed and an invoice will be sent for payment (see pricing for copy and printing services)    
• If you have multiple files that have different printing instructions, please use this form to submit each file separately with their specific requirements.
• After payment, your job will be processed and you will be notified when it is ready (up to 24-72 hours).
